The Fall Risk Factor Nobody Talks About: Divided Attention

Most fall-prevention advice focuses on the obvious: strengthen your legs, improve your balance, clear the clutter from your hallways. All good advice. All incomplete.
Here's what often gets left out of the conversation: a huge percentage of real-world falls don't happen when someone is standing still, or even just walking. They happen when someone is walking AND doing something else — answering a question, carrying a bag, glancing at a phone, reaching for a shelf. Researchers call this "divided attention," and a growing body of evidence points to it as one of the most overlooked fall risk factors in older adults.
Why divided attention matters more than balance alone
Think about the moments that have actually challenged your balance recently. Chances are, it wasn't while standing still in your kitchen. It was crossing a parking lot while checking for cars and holding a bag of groceries. It was stepping off a curb while answering your phone. It was reaching for something on a high shelf mid-conversation.
These "two things at once" moments are where a stumble is most likely to happen — not because your legs suddenly got weaker, but because your brain had to split its attention between the physical task and the mental one, and something had to give. Researchers refer to this skill — doing a physical task and a cognitive task simultaneously — as dual-tasking, and studies on aging and fall risk increasingly point to dual-task performance as a stronger predictor of fall risk than static balance tests alone. Put simply: divided attention is the broader phenomenon, and dual-tasking is the trainable, practiced version of it.
The problem with most balance training
Traditional balance exercises — standing on one foot, walking heel-to-toe — are valuable, but they're often practiced in ideal conditions: quiet room, full attention, nothing else going on. That's not how falls actually happen. If the only balance training you're doing happens in silence with your full focus, you're training a skill you'll rarely get to use in exactly that form.
The good news is that this is trainable, just like static balance and strength. Practicing movement and mental tasks together — on purpose, in a low-stakes setting — teaches your brain and body to share the job more efficiently. That skill transfers directly to the real situations where falls happen: the grocery store, the parking lot, the hallway with grandkids running past.
A simple way to start
You don't need special equipment to begin building this skill. Try this: next time you're walking through your house, count backward from 20 by threes. Or name a piece of furniture for each letter of the alphabet as you walk from room to room. If either of those makes you feel unsteady, that's useful information — it means this is exactly the kind of practice worth doing, ideally with support nearby at first.
As the skill improves, the mental task can get slightly harder, or the environment slightly less predictable — closer to the unpredictability of real life, where phones ring, questions get asked, and curbs appear without warning.
